What Is a DOT SAP Evaluation?

Substance Abuse Professional (SAP) evaluation is a required step in the Department of Transportation (DOT) return-to-duty process after a drug or alcohol rule violation.

The purpose of the evaluation is to determine what is needed for safe and compliant return to safety-sensitive work.

The SAP is responsible for:

  • conducting the required initial evaluation

  • recommending any necessary education or treatment

  • verifying compliance with DOT requirements

  • determining eligibility to return to safety-sensitive duties

The goal is simple: to help you complete the process and return to work safely and legally.

The Return-to-Duty Process

Step 1 — Initial SAP Evaluation

A confidential evaluation conducted remotely by a DOT-qualified physician to determine what is required for return to duty.

Step 2 — Education or Treatment

Completed with an independent provider in accordance with DOT requirements.
Many education or treatment services may be eligible for insurance coverage.

Step 3 — Follow-Up SAP Evaluation

Evaluates compliance with the recommended education and/or treatment plan and determines eligibility to return to safety-sensitive work.

Step 4 — Employer Return-to-Duty Determination and Testing

After the SAP determines eligibility, the employer makes the final return-to-duty decision and arranges the required return-to-duty drug and/or alcohol testing in accordance with DOT regulations.
Successful completion of employer-directed testing allows the employee to resume safety-sensitive duties.

Many individuals are able to return to work within one to several weeks, while others may require a month or longer, depending on the level of education or treatment recommended and employer testing timelines.

    • determines eligibility to return to safety-sensitive work

    This evaluation is scheduled after compliance is demonstrated and is billed separately at the time of scheduling.

  • If education or treatment is required, it is completed with an independent provider in accordance with DOT regulations.

    Important notes:

    • Services and fees are determined by the treatment provider, not the SAP

    • Many education or treatment services may be eligible for insurance coverage

    • The SAP does not provide treatment as part of the evaluation process

    Documentation of compliance with the recommended plan is required before the follow-up SAP evaluation can occur.

What to Expect After Scheduling Your SAP Evaluation

Step 1 — Complete Intake and Secure Your Appointment

After scheduling, you’ll receive:

  • a short secure intake form

  • the SAP evaluation agreement

  • an invoice for the initial evaluation

Your appointment becomes fully confirmed once intake forms and payment are completed.


Step 2 — Initial SAP Evaluation

You’ll meet privately with a DOT-qualified physician by secure video conference.
During this visit, we:

  • review the circumstances of the violation

  • answer your questions about the process

  • determine any required education or treatment

  • explain the next steps toward return to duty


Step 3 — Education or Treatment (As Required)

DOT regulations require the SAP to recommend at least some level of education and/or treatment in every case, based on the individual evaluation.
The level of education or treatment can vary significantly, and in some situations only a brief level of education may be required.

Required services are completed with an independent provider.
Many treatment services may be eligible for insurance coverage, depending on the provider and individual plan.


Step 4 — Follow-Up SAP Evaluation and Return-to-Duty

Once you’ve demonstrated compliance with the recommended plan, we conduct a follow-up SAP evaluation to determine eligibility for return to duty.

Your employer then:

  • arranges the required return-to-duty drug and/or alcohol test

  • decides when you may resume safety-sensitive work

  • SAP services are billed per required step in the DOT return-to-duty process:

    • Initial SAP Evaluation: $350

    • Follow-Up SAP Evaluation: $200

    If education or treatment is required, those services are provided and billed separately by an independent treatment provider and costs may vary.
    You pay only for the services required, with no bundled or hidden fees.

  • SAP evaluation fees are typically self-pay.
    However, education or treatment services may be covered by insurance, depending on the provider and individual plan.

  • After demonstrating compliance with the recommended education and/or treatment plan, the SAP conducts the required follow-up evaluation.

    If the SAP determines the employee has complied with the education and/or treatment requirements and is safe to resume safety-sensitive duties, the SAP will determine the employee is eligible to return to duty under DOT regulations.

    The employer then arranges the required return-to-duty drug and/or alcohol test and makes the final decision about when the employee may return to safety-sensitive work.

    DOT regulations also require the SAP to establish a follow-up testing and monitoring plan, which includes a minimum of six unannounced drug and/or alcohol tests during the first 12 months after returning to duty. Additional testing may be required based on the SAP’s clinical judgment.
